Audition for 2026
Clare and the Chocolate Nutcracker

 

By checking the photo release box, I give Orlando Community Arts (OCA) permission to photograph, film or record my child in rehearsal, performance and/or production.

I understand that OCA may use these images for publicity, advertising, websites, or for applications for grant funding. I give full permission and agree to waive all copyright and future considerations.

My agreement to perform is voluntary, and I do hereby waive all personal claims, causes of action, or damages against OCA, Board Members and the employees thereof, arising from a performance or appearance.

There is no participation fee to perform in Clare and the Chocolate Nutcracker.

All participants are required to encourage the sale of 15 tickets from friends, family members, co-workers etc.

All participants will adhere to the production rules and regulation when performing at rehearsals and at the Dr. Phillips Center for the Performing Arts.

All female dancers will wear hair in a bun (up).

All participants are required to attend rehearsals for their designated rehearsal time. All participants must wear appropriate attire which will include the designated 2025 t-shirt which the cast will purchase prior to the first rehearsal.

I declare that I am the parent or legal guardian of the above-named child. In the event my child is injured or should require medical attention, I hereby request that you contact me or our emergency contact. If we can’t be reached, I hereby authorize Orlando Community Arts, Inc., to secure necessary medical treatment for my child. I further acknowledge that I will be responsible for any medical or hospital fees or costs associated with my child’s medical treatment.

I assume all risks and hazards from participation in this production and hereby waive, release, absolve and indemnify and agree to hold harmless Orlando Community Arts, Inc., its organizers, sponsors, directors, volunteers, and participants for any claim arising out of accidental injury to my child. All visitors must follow the Dr. Phillips Center’s health and safety protocols. Please help keep each other safe.
